 Background:Anatomical double-bundle anterior cruciate ligament reconstruction (DB-ACLR) is being increasingly performed in middle-aged and elderly patients. The purpose of this study was to compare the clinical results of DB-ACLR in a group of middle-aged and elderly patients and a group of young patients.

 Method:A total of 91 reconstructed knees were assessed one year after the operation. In this study "middle-aged and elderly" was defined as 40 years of age and over. We divided the patients into two groups:a young group (72 knees) and a middle-aged and elderly group (19 knees) and compared the two groups in relation to the cause of the ACL injury, waiting period before the operation, Lysholm score, anterior laxity, results of the pivot shift test, meniscus injury, osteoarthritic change, and knee flexor and extensor muscle strength.

 Outcome:There were no significant differences between the two groups in waiting period before the operation, Lysholm score, anterior laxity, results of the pivot shift test, meniscus injury rate, and knee flexor and extensor muscle strength.

 Conclusion:We concluded that DB-ACLR is an effective method of treating middle-aged and elderly patients.
