　抗凝固療法によりヘパリン起因性血小板減少症(HIT)を呈した77歳の女性の症例を経験したので報告する．保存的治療でも改善せず4年後にセメントレス人工膝関節置換術を施行した．術後1日にD-dimer値，FDP値の上昇を認め周術期血栓予防にヘパリンを使用した．ヘパリン使用後に血小板減少を認め，HITと診断した．下肢静脈エコーで深部静脈血栓症(DVT)を認めたが，重篤な合併症は発症しなかった．HIT抗体は2回とも陰性であった．

　A 77-year-old woman was diagnosed with heparin-induced thrombocytopenia (HIT) after anticoagulation.　Cementless total knee arthroplasty was perfomed after 4 years of conservative therapy for osteoarthritis of the knee, and on postoperative day 1, we observed a rise in D-dimer and FDP values and performed heparinization to prevent postoperative thrombosis.　We made a diagnosis of HIT and thrombocytopenia after heparinization.　The platelet count improved in response to treatment with argatroban after immediately discontinuing the heparinization.

　Venous ultrasonography of the patient's lower limbs revealed deep vein thrombosis, but it did not cause any serious complications.　Antibody testing in HIT was negative twice.
