 A 77-year-old woman was diagnosed with heparin-induced thrombocytopenia (HIT) after anticoagulation. Cementless total knee arthroplasty was perfomed after 4 years of conservative therapy for osteoarthritis of the knee, and on postoperative day 1, we observed a rise in D-dimer and FDP values and performed heparinization to prevent postoperative thrombosis. We made a diagnosis of HIT and thrombocytopenia after heparinization. The platelet count improved in response to treatment with argatroban after immediately discontinuing the heparinization.

 Venous ultrasonography of the patient's lower limbs revealed deep vein thrombosis, but it did not cause any serious complications. Antibody testing in HIT was negative twice.
