脊椎内視鏡手術でサルベージが可能であった難治性腰痛とみなされていた腰椎多数回手術例（MOB）の1例

Efficacy of a Salvage Operation with a Spinal Microendoscope in a Multiply Operated Back(MOB) Patient with Intractable Low Back Pain Due to Untreated Lumbar Foraminal Stenosis. Case Report

　腰椎手術後の遺残症状は多様であるが，いわゆる腰痛に関しては有効な治療手段を講じることができず，漫然と保存的治療を継続していることが少なくない．今回われわれは，難治性腰痛に対して神経根ブロックを試みたところ，患者の満足する十分な除痛効果を確認できた腰椎多数回手術(MOB)症例を経験した．このため，神経根性腰痛と診断し，脊椎内視鏡を用いてサルベージ手術を追加したところ良好な結果を得た．われわれが経験したように，神経根性腰痛は確かに存在するため，難治性腰痛の治療に際しては，本病態の可能性も十分に検討しなければならない．

　We often encounter multiply operated back (MOB) patients with intractable pain.　Since no treatment for their pain has been established, we often continue palliative treatment.　We report a case of intractable low back pain due to untreated lumbar foraminal stenosis, in an MOB patient in which salvage was possible by using a spinal microendoscope.　Radicular low back pain must be always taken into consideration when treating intractable pain in MOB patients.
