変形性肘関節症に対して,複数肢位で撮影したCTデータから,衝突する骨棘を3次元的に同定するコンピューターシミュレーション技術を開発した.術前に衝突する骨棘を同定し,それを基に鏡視下手術を行った.9例に施行したところ,術中概ね予定通りの可動域を獲得し,屈曲は術前平均117°から125°へ,伸展は平均-31°から-11°へ,JOAスコアは平均62点から91点へと改善した.本手法は衝突に関与する骨棘を正確に同定し,効果的な鏡視下手術の遂行を支援する有用な方法と考えられる.
We evaluated the results of arthroscopic debridement of 9 degenerative elbows based on a preoperative dynamic assessment of impinging osteophytes based on 3-dimensional computer bone models created from computer tomography data. Mean extension improved significantly, from -31° preoperatively (range:-20° to -45°) to -11° postoperatively (range:-5° to -20°) (p<0.01). Mean flexion improved from 117° preoperatively (range:80° to 135°) to 125° postoperatively (range:110° to 135°) (p=0.03). The mean Japanese Orthopaedic Association elbow score was 91 points postoperatively, as opposed to 62 points preoperatively (p<0.01). Preoperative computer simulation allows surgeons to precisely recognize “impinging osteophytes”, which must be removed, thereby enabling arthroscopic debridement to be performed in an effective manner.
