経皮的内視鏡下椎間板ヘルニア摘出術の経椎間孔アプローチと椎弓間アプローチ併用によるL4/5 down-migrateヘルニア摘出

Percutaneous Endoscopic Lumbar Discectomy by a Combined Transforaminal and Interlaminar Approach for Far-Downward Migrated Disc Herniations at L4-5

　大きく移動しているヘルニアは経皮的内視鏡下椎間板ヘルニア摘出術(PELD)経椎間孔アプローチの適応外であるが，L4/5の大きく尾側移動しているヘルニアをL4/5の経椎間孔アプローチとL5/Sからの椎弓間アプローチを併用することでPELDで摘出し得た．腰仙前屈位で2つのアプローチの併用で，L5背側のほとんどの範囲に到達でき，また，ヘルニア基部を断っているので，容易に移動部分を摘出できる．MEDと比べ今回の方法は，非常に小さい侵襲で遂行できるため，有用であると考える．

　Although far-migrated disc herniations have not been an indication for percutaneous endoscopic lumbar discectomy (PELD) by the transforaminal approach, it was possible to remove far-downward migrated hernias at the L4-5 level by a combined approach：a transforaminal approach at L4-5 and an interlaminar approach at L5-S.　Use of the combined approach in lumbosacral flexion enables almost the entire dorsal surface of the body of L5 to be reached, and disconnected migrated parts are easily removed after resection of the nonmigrant parts of the hernia.　This method is far less invasive than MED.
