腰椎椎間板ヘルニアに対する経椎弓的内視鏡下ヘルニア摘出術の経験

Translaminar Microendoscopic Discectomy for Lumbar Disc Herniation

　椎間板高位より頭側に脱出した腰椎椎間板ヘルニア4例に対し，経椎弓的内視鏡下ヘルニア摘出術を施行した．術後，全例で症状の改善を認め，短期ではあるが良好な術後成績が得られた．経椎弓侵入法では従来の経椎弓間侵入法と比べて多裂筋，椎間関節，黄色靱帯が温存でき，最小限の椎弓切除で頭側脱出型ヘルニアの摘出が可能である．さらにその侵入法に内視鏡下手術を導入することで，より低侵襲な手術が可能であった．経椎弓的内視鏡下ヘルニア摘出術は頭側脱出型腰椎椎間板ヘルニアに対する有用な低侵襲手術法のひとつであると考えられる．

　Four patients with cranially extruded lumbar disc herniation were treated by translaminar microendoscopic discectomy (TLMED). Treatment was successful in all patients, with good improvement in symptoms. The translaminar approach enables preservation of the multifidus muscles, the yellow ligament, and the facet joints. The microendoscopic technique reduces invasion of the posterior structures, improves the safety of the operation, and permits detailed visualization of the anatomy. TLMED is a minimaly invasive procedure for the treatment of cranially extruded lumbar disc herniation.
