大腿骨頚部・転子部骨折患者の入院中での歩行獲得の有無が,自宅退院に与える影響を評価した.対象者は,2004年12月1日から2005年11月30日の期間に福島市内の1つの病院を受診した113名である.術後2週間以内での平行棒もしくは歩行器での歩行獲得の有無が自宅退院に与える影響を性別や年齢などを補正要因として,多変量解析で分析した.その結果,歩行が獲得できた場合に対して,歩行が獲得できない場合の自宅退院できない危険率は3.09(95%信頼区間:1.15-8.35)であった.すなわち,術後2週間以内での歩行獲得の有無は,自宅退院の可否を予測する因子の1つであると考えられる.
The purpose of this prospective study was to evaluate the influence of walking ability during their postoperative hospital stay on the discharge to home of hip fracture patients. A total of 113 patients were recruited from a hospital in Fukushima City between December 2004 and November 2005. Logistic regression analysis was used to examine the risk of not returning home at the time of discharge among patients who had not regained walking ability within two weeks after surgery in comparison with those who had. The odds ratio after controlling for potential confounding factors, including age and sex, was 3.09 (95%CI:1.15-8.35), suggesting that recovery of walking ability within two weeks after surgery is an important positive prognostic factor for home discharge.
