　当院において2003年9月から2005年10月までのインフリキシマブ発売早期に投与した関節リウマチ患者30症例に対して有害事象の発生率を調べた．7例（23.3％）に有害事象の発生があり，3例（10％）が有害事象のためインフリキシマブ投与中止となった．重要な有害事象として感染症と投与時反応がある．高齢，糖尿病合併，ステロイド使用例は感染症発症のリスクが高いと思われた．また，長期休薬後にインフリキシマブを再投与する場合，重篤なinfusion reactionが生じる可能性が高く注意が必要である．

　We evaluated adverse events during infliximab therapy for 30 patients with active rheumatoid arthritis (RA) between September 2003 and October 2005. A total of 7 adverse events (24.1％) occurred, in 3 of them were severe (2 infections, an infusion reaction). In 3 patients, infliximab therapy was discontinued because of adverse events. Advanced age, diabetes, and oral glucocorticoids resulted in high risk factor of the infection. The re-administration of Infliximab after prolonged cessation could increase the risk for severe infusion reaction.
