筋肉転移で発見された胃癌の1例

Gastric Cancer Detected by Skeletal Muscle Metastasis : Case Report

　抄録：びまん性に広がった筋肉転移を初発とした胃癌の1例を経験したので報告する．症例は51歳，男性．主訴は右大腿前内側部腫脹および疼痛である．当科初診時，右大腿前内側部に発赤を伴う腫脹，硬結を認め，血液検査所見では軽度の炎症反応を認めた．これらの所見より化膿性筋膜炎を疑い手術を行った．術中に排膿はなく，大腿筋膜は固く肥厚し，筋間の結合織および縫工筋は瘢痕化していた．生検組織の細菌培養は陰性で，初回の病理組織診断は結節性筋膜炎であった．広域スペクトルの抗生物質を術後1週間投与するも局所所見は全く改善せず，病理組織の再検討を行った．その結果，筋膜内中心に印環細胞型腫瘍細胞が認められた．原発巣検索目的で行った上部消化管内視鏡で進行胃癌が発見され，その筋肉転移と診断した．本症例では筋膜内中心に印環細胞型腫瘍細胞がびまん性に増殖したため，軟部組織炎症と紛らわしい臨床像を呈したものと考えられる．臨床像および画像所見が炎症性疾患を思わせるにもかかわらず，感染症が否定的な場合には，癌の筋肉転移も考慮する必要がある．

　We experienced a case of gastric cancer detected by skeletal muscle metastasis. The patient was a 51-year-old man with pain, redness and swelling of the medial side of his right thigh. We suspected pyogenic fascitis, and we performed a surgical incision and drainage of his right thigh. Although he was on antibiotics, the swelling of his thigh had been increasing rapidly. Histological analysis revealed skeletal muscle metastasis of the signet ring cell type carcinoma, and an advanced gastric cancer was detected by the gastroscopy. When infectious diseases are not found although we suspect inflammatory diseases, it is necessary that we consider the skeletal muscle metastasis of the cancer.
