語義失語症状で発症し，病初期から左側頭葉と頭頂葉に萎縮を認めた脳変性疾患の1例

A Case of Gogi Aphasia with Predominantly Left Temporal and Parietal Lobe Atrophy

　語義失語症状で発症した脳変性疾患の68歳右利き女性例を報告した。本例は，さらに計算障害と軽度の構成障害を有していたが，経過5年を経ても明らかな記銘力障害は認めなかった。頭部MRIでは，左側頭葉ならびに左頭頂葉に萎縮を認め，脳血流SPECTでは同部位に加え，左前頭葉の血流低下を認めた。語義失語で発症する脳変性疾患で，本例のように初期に頭頂葉の萎縮を呈した例は稀である。病変分布と失語像から，本例は近年提唱されている前頭-側頭葉に原発性の萎縮を有する前頭-側頭部脳萎縮症例に対する分類型では分類しがたい症例と考えられた。

We report a 68-year-old right-handed Japanese woman who had a history of progressive difficulty in understanding speech and naming. Neuropsychologi-cal examination presented Gogi (word meaning) apha-sia and impairment of semantic memory for some common objects. She also presented acalculia and mild constructional impairment. There was no evi-dence of impairment in elementary perception and motor skills. Her memory performance of visual task was within normal range. She had neither personality change nor behavioral disorder.
