A case of 42 years old male was presented who suffered from repeated subarachnoid hemorrhage initially resulted from a ruptured aneurysm of the anterior communicating artery and later from a glioblastoma multiforme of the bilateral frontal lobes.

First attack occured with drowsiness and severe headache, and an anterior communicating aneurysm was successfully clipped five months later. The post-operative recovery had been uneventful. Two months later there was second episode of a subarach-noid bleeding with drowsiness, stiffneck and 1- hemiparesis. At this time, bilateral frontal mass lesion was apparent on carotid angiography and unexpectedly a glioblastoma multiforme containing hemorrhagic lesions was removed.

Another subarachnoid bleeding following com-plete operative occulsion of aneurysm is rare, but possibility of other lesion causing hemorrhage such as tumor must be considered.
