A rare case of skull metastasis of malignant struma was reported. A 62-year-old female who had been operated for struma two times ( 2 and 4 years ago) admitted because of semicomatous state and hemiparesis on her right side.

Skull defect was found at the left temporo-parietal region on a plain film and the left carotid angiogram revealed the tumor stain which mainly grew intra-cranial space and was fed through the external carotid artery at the same region. Skull metastasis was totally removed, but the postoperative scintig-ram by radioiodine revealed other metastasis at the skull, the both lung, the 12th chest vertebrae and the pelvis. Operation was performed for the chest vertebrae and the inner or the outer radiation therapies by 131I or 60Co were done for the others. Patient is now alive and well about 2 years after last operation.
