We reported six cases of the intra- and/or supra-sellar aneurysms which was associated with visual field defect.

Five of them were aneurysms of the anterior com-municating artery, and one was of the internal carotid artery. All of the cases had visual field defect and visual impairment. These aneurysms were larger ones, being the largest 25×20×20 mm in size, and were compressing the visual tract.

Intracranial surgery for these aneurysms is difficult, because they are seated at the "dangerous location (Pool)".

Jefferson, and White convinced that "there was only one useful treatment for these cases, and that was carotid ligature".

But on each case of us, intracranial direct attack was successfully performed under hypothermic anesthesia. Four aneurysms of the anterior com-municating artery were extirpated, one of the anter-ior communicating artery and one of the internal carotid artery were trapped.

Following surgery, visual field defect and visual acuity were improved in every case.

All of the patients returned to the previous oc-cupation, and are enjoining their every day lives.
