 Several case studies have suggested genetic factors underlying the pathogenesis of neuroleptic malignant syndrome (NMS) which is a severe side effect of psychotropic drugs. Cytochrome P 450 IID 6 (CYP 2 D 6) plays an important role in the hepatic metabolism of variousneuroleptics and antidepressants. We detected a mutation in the CYP 2 D 6 gene in 4 patients who previously exhibited episodes of NMS. The neuroleptics which induced NMS were the substrates of CYP 2 D 6. Polymerase chain reaction and restriction fragment length polymorphism analysis revealed that all subjects were homozygousfor a mutated CYP 2 D 6*10 allele indicating impaired drug metabolism. This trait may contribute to susceptibility to NMS.
