We described a 41-year-old woman (Case T. D.) with herpes simplex encephalitis, in which significant recovery of amnesic syndrome was noted despite bilateral hippocampal damage. In the acute phase of her illness, marked anterograde amnesia for both verbal and non-verbal materials and experiences of Déjà vu concerning other people were found without recognizable retrograde amnesia and general intellectual loss. During her 4-week hospitalization, Déjà vu experiences disappeared, and her memory impairments recovered to subnormal level. At 20 months after discharge, the only evidence of her memory problems was a forgetfulness in daily life. Magnetic resonance imaging in the acute phase showed inflammatory changes in the limited area of the bilateral hippocampal formation and adjacent structures. On a MRI at 20 months after discharge, the bilateral hippocampal formation showed mild but distinct atrophy. These observations suggest that hippocampus is related to memory, but other medial temporal lobe structures also have an important role in memory. The experience of Déjà vu concerning people in this patient may be related to preserved face recognition systems despite the involvement in the medial temporal memory systems.
