Fluvoxamine追加投与開始後に軽躁状態を呈したうつ病の1男性例

A Case Study of A Depressed Male Displayed Hypomania by Additional Use of Fluvoxamine to Trazodone

【抄録】　うつ病にて入院中，従来の抗うつ薬による治療に加えfluvoxamineを投与したところ軽躁状態を呈した55歳の男性例について，SSRIによる躁転に関するこれまでの報告を踏まえ，心理行動面での変化を含めて考察した。入院7か月後にsulpiride，trazodoneに加えてfluvoxamine 50mg/日投与を開始したところ，表情ににやつきが現れ，服薬を忘れたり，無断離院，帰院時間の無視などの規則違反が頻発し，パチンコやアルコールへの傾倒がみられた。投与開始後34日で投与を中止したところ6日後には改善した。本症例ではfluvoxamineの追加投与が軽躁状態の発症に関与した可能性が考えられた。

　The authors discussed behavioral and psychological changes in a 55-year-old male who displayed hypomania after an addition of fluvoxamine to other antidepressant medications. When fluvoxamine 50 mg/day was prescribed in addition to sulpiride and trazodone 50 mg/day at 7 months after admission, grinning and violation of ward rules occurred repeatedly. And he became addicted to gambling and alcohol. In six days after cessation of fluvoxamine his condition was remitted.

　We speculated that addition of fluvoxamine accounted for his hypomania.
