 Interferon (IFN) therapy can induce psychiatric symptoms such as insomnia, mood disorders, psychosis, and consciousness disturbance. With the increasing use of IFN therapy, its psychiatric symptoms have been recognized as a major concern since they sometimes lead to suicide. Although psychiatric symptoms induced by IFN therapy are reported to be closely related to previous psychiatric history, other risk factors have not been identified. In the present study, we evaluated the relationship of the patient's personal and family history of psychiatric disease, age, and sexuality with the psychiatric symptoms induced by IFN therapy. Furthermore, the relation of the Temperament and Character Inventory (TCI) subscale scores was also examined. This prospective cohort study included 114 consecutive patients with chronic hepatitis C who received IFN therapy between June 2009 and June 2014. Of these 114 patients, 30 developed psychiatric symptoms after 2 weeks of IFN therapy;17 patients experienced insomnia, 8 experienced mood disorders, 4 experienced neurosis, and 2 experienced psychosis. Three patients needed psychiatric hospitalization. Logistic regression analysis revealed that previous psychiatric history (OR=6.632, 95% confidence interval (CI)=2.088-21.064, p<0.005) and the ” harm avoidance” score in TCI (OR=1.204, 95% CI=1.049-1.380, p<0.01) were useful as predictive factors for psychiatric symptoms induced by IFN therapy. Analysis of receiver operating characteristic (ROC) curves revealed that the cutoff value for “harm avoidance” was 15.5 (on a 20-point scale). Our findings indicate that in addition to previous psychiatric history, the “harm avoidance” score in TCI can be a useful predictive factor for psychiatric symptoms induced by IFN therapy.
