神経心理学的検査による分裂病性痴呆とアルツハイマー型老年痴呆の比較

Comparison between Schizophrenic Dementia and Dementia of the Alzheimer Type with Neuropsychological Tests

　抄録　いわゆる分裂病性痴呆と老年痴呆の異同を検討するため，長谷川式痴呆診査スケールによりほぼ同程度の知的機能の低下を有すると判定された慢性分裂病者群ならびにアルツハイマー型老年痴呆患者群に対して，概念形成・推理，絵画配列，およびRavenのColored Progressive Matricesの3種類より成る問題解決課題，ならびに記憶課題により構成される我々独自のテストバッテリーを施行した。その結果，老年痴呆群においては記憶障害が目立つのに対し，いわゆる分裂病性痴呆では問題解決能力の低下が比較的高度であることがわかった。なお，両群における問題解決能力の低下の差異を最も有効に検出するためには，至適の課題難易度が要求されるように思われた。また，老年痴呆患者の問題解決能力の低下には記憶障害がかなり関連するのに対し，分裂病者の問題解決能力の低下は記憶障害とはほぼ無関係であると判定された。

　We previously reported on the relationship between the defect of temporal orientation and intellectual deterioration in chronic schizophrenics^11~13). In the present study we compare intellectual deterioration in chronic schizophrenics to that in dementia of the Alzheimer type (DAT) using neuropsychological methods.
