 The anti-manic, anti-depressive and prophylactic effects of carbamazepine (CAZP, Tegretol-Ciba Geigy) on the endogenous manic-depressive psychotics (MDP) were investigated on 90 cases. The dosage of the drug ranged from 200 to 1200mg daily (usually 400 to 600mg).

 The effect of CAZP on 64 manic states was: markedly effective 14 (22%), effective 8 (13%), slightly effective 13 (20%), ineffective 30 (47%), whereas the effect on 9 depressive states was: markedly effective 2 (22%), slightly effective 1 (11%), ineffective 6 (67%). No definite relationship was found between the kinds of drugs in combination use and the effect of CAZP. No significant difference of the CAZP effect was found in the 4 patient groups with different age of onset of the illness, nor among the 'continuous' 'frequent' and 'periodic' type cases.

 The prophylactic effect of CAZP on 51 cases in which the assessment of the effect on manic episode was possible was: markedly effective 18 (35%), effective 16 (32%), ineffective 17 (33%), whereas that for depressive episode was; markedly effective 16 (38%), effective 11 (26%), ineffective 15 (36%), and the result of the general assessment on each patient was 18 (35%), 16 (32%), 17 (33%), respectively. The prophylactic effect of CAZP tended to be more remarkable on the cases with early onset (before 20 years) and of 'continuous' type of course.

 The prophylactic effect on MDP of CAZP did not parallel that of lithium salt, but CAZP may be effective on cases which did not respond to lithium salt and vice versa. Side effects such as ataxia, dizziness, exanthema, drowsiness and headache were observed in 19 (21%) cases, and CAZP was discontinued in 4 of them. The mechanism of effectiveness of the CAZP on MDP was discussed.
