We retrospectively investigated the use of brief-pulse current wave and sine-wave current in modified electroconvulsive therapy (ECT) at the Kure Medical Center. Thirteen patients received only pulse-wave ECT (the pulse-wave group), 12 patients required a switch from pulse-wave to sine-wave ECT (the switching group), and 11 patients received sine-wave ECT (the sine-wave group). Approximately half the patients treated with pulse-wave ECT required to be switched to sine-wave ECT because of inadequate seizures. There was no difference between the 3 groups with respect to the extent of improvement effected by ECT and the applied dose of psychotropic drugs, but the average numbers of ECT sessions, side effects, and days of hospitalization in the switching group were significantly higher than those in the other 2 groups. Therefore, it is necessary to conduct further studies on methods to reduce the number of cases requiring the switch from pulse-wave ECT to sine-wave.
