初老期・老年期発症の精神障害として経過した後に前頭葉変性型の前頭側頭型痴呆が疑われた2症例

Two Cases with Frontotemporal Dementia of the Frontal Lobe Degeneration-Type, Suspected after a Clinical Course of Presenile or Senile Psychiatric Disorders

抄録

　初老期および老年前期発症の非器質性精神障害として診断・加療されていたが，一連の状態像の変化により，器質性精神障害と診断されるに至った2症例について報告する。症例1は妄想性障害，症例2は精神病性の特徴を伴ううつ病性障害として加療されるも，病初期より継続していた訴えは次第に切迫感を欠いた単調なものへと変化していった。同時に常同行為・発動性低下が顕在化し，軽度の認知・記銘力の低下を認めるなど，器質性精神障害の存在を疑わせる病態を呈するに至った。形態画像上は，当初は目立たなかった両側前頭葉および側頭葉前方部の萎縮が次第に明らかとなり，機能画像上は前頭葉，次いで側頭葉の血流低下が認められた。これらの器質性精神障害を示唆する所見は進行性であり，正常老化の範囲を越えるものと考えられた。このような症例が，前頭側頭型痴呆(FTD)の前頭葉変性型に相当する可能性につき考察した。

　We report two clinical cases with frontotemporal dementia (FTD) suspected to be due to frontal lobe degeneration (FTD)-type. At the early stage of their clinical courses, these patients had been diagnosed as having presenile or early senile psychiatric disorders such as delusional disorder and major depressive disorder with psychotic features, At the advanced stage, they showeb progressive frontal lobe dominant atrophy on brain SPECT and stereotyped complaints and behavior as wellas mild cognitive deficit. These features are progressive and may be consistent with those of the FTD of the FLD-type.
