　Flavobacterium IIBが病原菌と同定された化膿性心外膜炎の1例を報告する．患者は81歳女性，主訴は発熱．1999年10月2日より38℃程度の発熱がみられ，近医受診し，胸部CTと心エコーで両側胸水と心嚢液貯留が認められた．抗生剤を投与され，やや解熱するも，胸水と心嚢液の増悪がみられ，10月5日当科に紹介され，緊急入院となった．心嚢液は滲出性でFlavobacterium IIBが検出された．尿培養でも同菌が同定され，尿路感染症から播種したと考えられた．持続心嚢ドレナージと抗生剤投与で心嚢液と胸水は消失し，全身状態も軽快した．化膿性心外膜炎は抗生物質の発達により，稀な疾患となっている．病原菌はブドウ球菌などのグラム陽性菌が大半を占めていたが，近年グラム陰性桿菌の頻度が高くなってきている．しかし，Flavobacterizamが本疾患の病原菌として同定された報告はみられない．

An 81-year-old woman was admitted to our hospital because of fever, on Octorber 3, 1999. An echocardio-gram and computed tomography demonstrated a mas-sive pericardial effusion and bilateral pleural effusion. Pericardial fluid analysis revealed gram negative bacilli on smear and Flavobacterium BB on culture. The same organism was isolated from urine culture. Antibiotic therapy with IPM/CS and continuous pericardial drain-age were effective. Pericardial and pleural effusion disappeared in 1 month. Purulent pericarditis is rare because of the advance of antimicrobial therapy. Peri-carditis is caused by a wide variety of organisms. However Flavobacterium group has not been reported so far as a pathogen of pericarditis.
