　心房中隔欠損症に部分肺静脈還流異常が合併することはよく知られている．今回，経食道心エコー図が術前術後の状態の検討に有用であった1例を経験した．症例は31歳，男性で，外来で心房中隔欠損症を認めた．心臓カテーテル検査で右上葉枝の部分肺静脈還流異常（PAPVR）の合併を認めた．術前の経食道心エコー図で非典型的な2次孔欠損を確認した．術後，一時右上肢の腫脹が出現したため，経食道心エコー図を施行した．経食道心エコー図では，PAPVR修復後の右房と左房へのconduitが明瞭に観察でき，同修復部位の閉塞，狭窄は認めなかった．経食道心エコー図の所見から観血的処置の必要はないと判断し，保存的治療を継続したところ，以後順調に軽快した．術後早期の心房内conduitの状態を検討するのに経食道心エコー図は侵襲も比較的少なく，狭窄などの評価に有用であると考えられた．

This report describes the usefulness of transeso-phageal echocardiography (TEE) for evaluation of post operative conduit in a case of atrial septal defect (ASD) with partial anomaly of pulmonary venous return (PAPVR).

A 31 year-old male was admitted for an operation on ASD and PAPVR. Preoperative cardiac catheterization showed PAPVR of the right upper pulmonary vein and ASD with 35% L-R shunt. A few days after the operation, the patient's right upper extremity began swelling and the patient complained of pain. To check the conduit, firstly, transthoracic echocardiography was performed.
