頭蓋外高位内頸動脈瘤の手術治験例—両側下顎骨亜脱臼法による術野展開の経験

A Successful Graft Replacement for High Extracranial Internal Carotid Aneurysm:Experience of submandibular subluxation technique for carotid exposure

　頭蓋外内頸動脈瘤は他の末梢性動脈瘤に比し稀である．今回，頭蓋外高位内頸動脈瘤に対して両側下顎骨亜脱臼法による術野展開のうえ，単純遮断下に瘤切除および血行再建術を行い良好な結果を得た．症例は右下顎部腫脹，咽頭不快感を主訴とする51歳，男性である．頸部CT検査では下顎角の高さで最大径45mmを呈する右内頸動脈のbull's eye像を認めた．Matas'　test下の脳血管造影検査では健側よりのcross-fillingは良好であった．単純遮断による血行再建術は，症例を選択したうえで施行すれば有用な術式であると考える．また，両側下顎骨亜脱臼法は頭蓋外高位内頸動脈の露出に際し，簡便かつ有用であった．

A case of extracranial internal carotid aneurysm extending up to the first cervical vertebra was success-fully operated by a simple technique. The operative field was exposed by bilateral mandibular subluxation using maxillary-mandibular arch bars, and the aneurys-mectomy and graft replacement were carried out using simple clamp under administration of brain protective substrates. The continuous monitoring of both electroenchephalogram (EEG) and oxygen saturation in jugular vein showed no evidence for cerebral is-chemia.
