　硝酸薬類似の血管拡張作用を有するβ遮断薬nipradilol 12mgの労作性狭心症における有効性を確認するとともに，運動負荷時心拍数-ST偏位解析を用いて，その作用機序について考察することを目的とした．9例の慢性安定型労作性狭心症を対象に，症候限界性の多段階漸増負荷試験を観察期と投与8週間後に行い，同時にHR-ST解析を行った．狭心発作回数は2週目より有意に減少し，安静臥位時の心拍数は有意に減少したが，血圧は不変であった．最大酸素摂取量は13.7±1.1ml/kg/minから19.3±2.5ml/kg/minへと著明な増加を示し，最大ST下降は1.72±0.45mmから1.17±0.19mmへと有意に改善した．HR-ST解析ではΔST/ΔHRとΔST/ΔDPは軽度ながら改善傾向を示した．β遮断薬の単独投与ではΔST/ΔHRとΔST/ΔDPの有意な増加を示すことから，nipradilolの最大ST下降改善効果にはβ遮断作用に血管拡張作用が加わっているものと推察された．

This study was performed to confirm the usefulness of 12 mg nipradilol, a β-blocker with a vasodilator action similar to that of nitrate-type drugs, in the treatment of effort angina pectoris. It also discusses the mechanism of the action of this drug when using the exercise test heart rate-ST displacement analysis. The symptomthreshold multistage graded exercise test was performed using nine patients with chronic stabilized effort angina pectoris after an observation period and an 8-week administration period and HR-ST analysis was performed simultaneously.
