　【目的】体表面加算平均心電図（SAECG）による術後右脚ブロックの部位に関する研究．【対象および方法】右脚ブロックを有する先天性心疾患術後患者35名（ファロー四徴症20名，心室中隔欠損症15名）を体表面電位図により右脚本幹ブロック12名および右脚末梢ブロック23名に分類し，vector magnitude法で求めたSAECG所見と比較検討した．【結果】右脚本幹ブロックの症例は，①創tered QRS（f-QRS）durationが160 msec以上，②frag-mented signal（FS）数が11個以上，③f-QRSパターンがwhold type，の3所見を示すことが多かった．これら3所見のうち，右脚本幹ブロックに対するsensitivityはf-QRSパターンが，specificityはFS数がもっとも良好な値を示した．一方，二枝ブロックに特徴的なSAECG所見は見出せなかった．【結語】SAECGは先天性心疾患術後右脚ブロック部位の補助診断法として有用と思われた．

Right bundle branch block (RBBB) type was inves-tigated using signal-averaged electrocardiogram (SAECG) in 35 RBBB patients after surgical repair of ventricular septal defect and tetralogy of Fallot. The type of RBBB was first identified by body surface mapping; 12 patients had proximal RBBB, and 23 had peripheral RBBB. The control group consisted of 20 healthy volunteers.
