Phrenic nerve paralysis (PNP) following open heart surgery is one of the complication of hypo-thermic and topical cardiopleagia, although we rarely experience as a clinical difficulty after the opera-tion.

Recently, we experienced two severe of PNP following open heart surgey. Each patient receive mechanical ventilation for 25 days and 35 days, res-pectively, and they required prolonged intensive care and long-term hospitalization.

We retrospectively studied 158 patients undergo-ing open-heart surgery to evaluate the incidence of PNP which showed 36.1%. This complication was likely to occur in the patients who had A-C bypass in our hospital in which we prefered to the use comparatively large volume of ice-slush.

We adopted the use of Cardiac Insulation Pad (CIP) for prevention of PNP. Following the use of CIP in the group of 65 consecutive patients, only 7. 7% showed evidence of PNP
