 The effects of exercise training with exercise intensity below anaerobic threshold (AT) and the significance of AT in exercise prescription for coro-nary heart disease patients were studied. Symptom limited treadmill exercise test were performed be-fore and after 4.1±1.4 months of exercise training with exercise intensity below AT (from 48.6% to 54.0% of maximal heart rate) in thirteen coronary heart disease patients. In five cases (group 1), the AT was recognized in the exercise test before and after exercise training. In other five cases (group 2), the AT was recognized only in the exercise test after exercise training. In three cases (group 3), the AT was not recognized in the exercise test before and after exercise training. In group 1, the maximal oxygen uptake (VO2mmax) increased from 21.6ml/kg/ min to 28.4 ml/kg/min (p<O.05) and the oxygen uptake at AT (VO2m-AT) increased from 17.1ml/kg/min to 20.5ml/kg/min (p<0.01) after exercise training. In group 2, the VO2max increas-ed from 15.5ml/kg/min to 20.5 ml/kg/min (p< 0.05) and heart rate and systolic blood pressure at submaximal exercise decreased significantly after exercise training.

 It appears that these improvement of exercise tolerance were mainly due to the peripheral adapta-tion. The improvement of VO2m-AT is induced even by the exercise training with exercise intensity below AT and the AT is not a consistent guideline to determine the optimal exercise intensity in the exercise prescription for coronary heart disease patients.
