In order to determine the incidence and pattern of angina as a premonitory symptom of acute myocardial infarction and study in relation to acute complication, prognosis and the findings of coronary angiography, 489 patients with acute myocardial infarction were questioned shortly after hospital admission about the presence and pattern of chest pain prior to onset of infarction.

151 of 414 patients in new infarction (36,5%) and 27 of 75 patients in re-infarction (36.0%) had no angina before infarction. 139 patients in new infarction (33.6%) and 17 patients in re-infarction (22.7%) had a history of new angina within one month of myocardial infarction. 124 patients in new infarction and 31 patients in re-infarction had a history of chronic angina and of these, 45 patients in new infarction and 12 patients in re-infarction noticed worsening of their symptoms in one month prior to infarction.

Hospital motality rate was high in a group ofsudden onset without angina (group I) and a group of new angina within one month prior to infarction (group II) and low in a group of chronic angina.

Incidence of cardiac and septal rupture was more high in a group of new infarction than in a group of re-infarction. In a group of new infarction,incidence of these rupture seems to be high in group I and group II.

In relation to the degree of coronary stenosis, in-cidence of normal coronary was high in group I, and incidence of double or triple vessel was high in a group of chronic angina.
