Right ventricular infarction was manifested by low-output syndrome in some patients with acute myocardial infarction. Right ventricular infarction occurs primarily in the setting of acute infarction of the infero-posterior left ventricle or the interven-tricular septum.

Marked right ventricular dysfunction due to right ventricular infarction is a possible mechanism underlying the low-output syndrome.

This study was undertaken to determine whether or not right ventricular dysfunction can produce a low-output state in the absence of primary left ventricular dysfunction.

Eight dogs were anesthetized and the pericardial cradle was constructed after the median thorac-otomy. Isolated right ventricular ischemia was produced by ligation of the right coronary artery (RCA). Then, 120 minutes after RCA ligation, ligation of the left anterior descending artery (LAD) was added.

Heart rate did not change significantly during the course of an experiment. After ligation of RCA, right ventricular systolic pressure decreased signifi-cantly by 15.4%, and cardiac output by 34.6% with no changes in right and left ventricular end-diastolic pressure and aortic, pressure. LAD ligation in addition to RCA ligation did not produce further decrease in cardiac output, and aortic pressure.

The results of this study support that a functional right ventricle is important to maintain cardiac output.
