A 30-year-old woman, undergoing neurosurgery for removal of cerebellar tumor in the sitting position, suffered three episodes of air embolism and a total amount of 15 ml of air was aspirated through the Swan-Ganz catheter positioned in the right ventricle. At the end of surgery the patient awoke immediately. However, mild hypoxemia was observed for a few days and right hemiparesis developed. Cerebral air embolism was considered as a cause of this neurological sequella.

Acute pulmonary edema and neurological complication following pulmonary air embolism were discussed.
