完全房室ブロックにおける心室overdrive testの有用性についての臨床的・実験的研究

Clinical and experimental studies on the usefulness of ventricular overdrive test in complete AV block

　完全房室ブロックにおけるペースメーカ装着は，1） Adams-Stokes発作があるとき，2）徐脈のために心室細動など致命性な頻拍性不整脈を起しやすくなっているとき，3）徐脈のために心不全その他の諸症状があるとき，に適応があると考える。ところでこのうちの前2者は可能性を未然に知って適切な処置をとることが要求されるものである。完全房室ブロックでAdams-Stokes発作をみるのは，普通房室伝導が一旦回復し，再び途絶したときに下位中枢がすぐに出現せず心停止の状態となるためであろう。したがって心室リズムのさいに心室をペーシングして急に中絶したときの心室収縮の現われ方をみることにより，Adams-Stokes発作の起る可能性を推測できるはずである。そこで著者らはこの心室のoverdrive testとでもいうべきものの有用性を知る目的で以下の臨床的・実験的検討を試みた。

The duration of ventricular asystole resulting from AV block can be speculated by studying the duration of ventricular asystole after discontinuing ventricular driving. The present clinical and experimental studies were designed to assess the usefulness of the ventricular overdrive test in predicting Adams-Stokes attacks in complete AV block patients.

The overdrive test was carried out in 10 patients with complete AV block, including 3 with A-H block, 3 with BH block and 4 with H-V block.
