冠動脈バイパス術における大伏在静脈採取法の違いによる合併症の検討―VasoViewを用いた内視鏡的大伏在静脈採取法の経験

Three Methods for Saphenous Vein Graft Harvesting during Coronary Artery Bypass Grafting：Initial experience of endoscopic saphenous vein harvesting using the VasoView system

当院では冠動脈バイパス術の大伏在静脈採取法としてグラフト全長にわたって切開する全長切開法，皮膚切開を分割する間歇切開法，VasoViewを用いた内視鏡的採取法の3法を行ってきた．今回，各採取法における静脈剥離部の合併症を検討した．感染，創哆開，リンパ漏などの合併症そのものには統計的有意差はなかった．しかし，3法における静脈剥離部の合併症について差がないとすれば短い皮切が美容上有利であると考えられた．また，VasoViewは冠動脈バイパス術時の大伏在静脈を内視鏡的に剥離採取するシステムであるが，本邦ではまだ使用経験が少ない．当院では2004年10月からこのシステムを導入し，初期の使用経験を従来の採取法と比較して考察した．

Complications encountered while carrying out greater saphenous vein harvesting during coronary artery bypass grafting were compared. The comparison was made between ordinary open vein harvest, minimally invasive open vein harvest, and endoscopic vein harvest. No significant differences in complications were observed between these groups. The shorter incision in endoscopic harvest was considered advantageous from an esthetic perspective.

　We report our initial experiences with endoscopic saphenous vein harvesting using the VasoView system. Endoscopic harvest technique was the most useful procedure for esthetic results.
