要旨●自己免疫性胃炎(AIG)は胃癌の発生母地となり,H. pylori感染の減少に伴い,胃癌の背景疾患としての重要性が相対的に高まってきている.自験AIG 194例について遡及的に検討を行ったところ,胃癌合併は36例(18.6%)に47病変認められた.胃癌合併例と非合併例を比較すると,胃癌合併例のほうが高齢で,内視鏡所見では固着粘液陽性率が低く,生検標本のupdated Sydney systemによる評価で前庭部腸上皮化生スコアが高かった.AIG合併胃癌の特徴は0-IIa型の早期癌で,胃型形質の分化型腺癌が典型例と考えられた.AIGには胃腺腫も26例(13.4%)に認められ,胃癌のうち4例4病変は胃腺腫として経過観察していた症例が悪性化したもので,腺腫にも注意を要すると考えられた.
AIG(autoimmune gastritis)is a recognized cause of gastric cancer. Owing to the decline in Helicobacter pylori infection rates, its significance as a background disease for gastric cancer has relatively increased. In a retrospective study, in our department, comprising 194 patients with AIG, gastric cancer complications were documented in 36(18.6%)patients with 47 lesions. Patients with complicated gastric cancer were older, had a lower rate of positive endoscopic findings for adherent mucus, and higher intestinal metaplasia scores on biopsy specimen assessment as per the Updated Sydney system compared with those having uncomplicated gastric cancer. Gastric adenomas were also found in 26(13.4%)patients with AIG ; 4 lesions in four of the gastric cancers were malignant transformations of cases that had been followed up as gastric adenomas, indicating that adenomas also warranted attention.
