早期胃癌の深達度診断─拡大観察の現状

Current Clinical Feasibility of Magnifying Endoscopy for Predicting Early Gastric Cancer Invasion Depth

要旨●目的：拡大観察が深達度診断に応用され有用性が検証されているかを求めた．方法：一定のキーワードを用いPubMedで該当論文を検索し対象に用いた．対象文献のレビューを行いエビデンスレベルを求めた．さらに頻用されている所見について再現性を求めるために遡及的検討を行った．結果：3編の該当する論文が抽出された．拡大観察が深達度診断に有用であるということを高いエビデンスレベルをもって検証した研究成果を示した論文を認めなかった．コンセンサスも不十分であった．筆者らの遡及的検討の結果，再現性のある結果は得られなかった．結論：拡大観察の早期胃癌の深達度診断に対する有用性は未確立である．

　Aim : We examined the feasibility of using magnifying endoscopy to determine EGC（early gastric cancer）invasion depth.

　Method : We searched the literature for articles describing the feasibility of magnifying endoscopy in predicting EGC invasion depth using PubMed. Further, we re-evaluated the reproducibility of previous report findings using a retrospective study.

　Results : Three original articles were found and included in our study. By reviewing these articles, we found a lack of consensus and high quality evidence demonstrating magnifying endoscopy as accurate in determining EGC invasion depth. Therefore, in our retrospective study, no reproducibility of previous findings was obtained.

　Conclusion : The feasibility of magnifying endoscopy for predicting EGC invasion depth has not been demonstrated due to a lack of high quality evidence.
