超音波検査で腫瘍の自然脱落が観察できた胆囊pm癌の1例

Early Carcinoma of the Gallbladder Invading the pm which Exhibited Spontaneous Self-detachment, Report of a Case

要旨　超音波検査法（US）により胆囊内に隆起性病変が指摘され，術前検査を進めていく間に著明な形態変化を来し，手術により脱落した腫瘍組織と凝血塊がそのecho sourceと確認された胆囊pm癌の症例を経験したので報告した．症例は64歳の女性で，主訴は右季肋部痛である．近医を受診しUSにて胆囊隆起性病変を指摘され，精査のため当科に紹介入院となった．USによる経過観察を行ってみると，隆起の形態が著明に変化し胆囊壁から脱落せんとする画像が捉えられた．手術の結果，脱落した腫瘍組織と凝血塊が証明された．組織学的には筋層にわずかに浸潤した乳頭管状腺癌であった．自然脱落という胆囊癌の自然史を実証する貴重な症例と考え報告した．

　A 64 year-old woman was admitted to our department for close examination of a polypoid lesion of the gallbladder.

　During preoperative examinations, ultrasonography revealed spontaneous self-detachment of the polypoid lesion (Fig. 3). Surgery confirmed the self-detached tumor and a clot (Fig. 7). Histologically, the tumor was a papillotubular adenocarcinoma invading the proper muscle layer (Fig. 8, 9). This case is considered to be valuable for understanding the natural history of gallbladder carcinoma.
