最近われわれは,下痢で来院した19歳の男性に消化管造影検査を行ない,大腸のみならず,食道,胃,十二指腸にもポリープが認められたが,既に報告されているいずれの大腸ポリポーシスの範疇にも属さないと思われる症例を経験したので若干の文献的検討を加え報告する.
A 19-year-old male patient visited Kitasato University Hospital because of diarrhea. No family history of intestinal polyposis, abnomal laboratory data, and other accompanying lesions were detected. Gastrointestinal X-ray series and endoscopic examinations disclosed generalized gastrointestinal polyposis. Numerous tiny polypoid lesions were detected in the esophagus, stomach, duodenum, and the colon. The microscopic findings of these polypoid lesions showed slightly inflammed lamina propria covered by almost normal epithelium.
The pathogenesis of these inflammatory changes remains obscure, but at least they are not interpreted as a cause of polypoid lesions. If these changes be considered to be simply incidental, the polypoid lesions could be interpreted to be hypertrophic mucosal tags. However, no single case of generalized gastrointestinal polyposis caused by mucosal tags alone has ever seen in the previously reported literature. Therefore, at present this case cannot be categorized clinicopathologically into any type of gastrointestinal polyposis.
This patient has a very interesting type of generalized gastrointestinal polyposis, which may become a new clinical entity.
