  • Look Inside

As peroral fiberscopic examination of the papilla of Vater, recently exploited, has still many difiiculties, a scope better suited for this purpose is now being developed with due regard to the following points: (1) the fiberscope should be very flexible, with (2) its visual window opened in the side wall at its tip.

A very pliable duodenal fiberscope has thus been devised 7mm in diameter and 1,250 mm long accompanied with a light guide system. Of 6 cases tried with this scope for the examination of the papilla of Vater, the duodenal mucosa was visualized in 4, and of these, successful visualization of the papilla was obtained in 2. These two cases are described in this paper.

The above-mentioned clinical experience revealed a difficulty in obtaining constant and steady visualization of the papilla, so that some mechanical dilatation of the duodenal wall had to be contrived. In order to hold a clear visual distance, around the tip of the scope was mounted a cage, which is easily dilated by pulling a steel wire at hand after the scope has been introduced into the duodenum.

The scope with the cage has been tried experimentally with dogs, resulting in much better visualization of the papilla with constant vision of the duodenal lumen.

Further developemnt of the duodenal fiberscope is now under research.
