私の胃体部大彎正面像の描出法

HOW T0 DRAW UP THE FACE-ON VIEW OF THE GASTRIC CORPUS ON THE GREATER CURVATURE

Ⅰ．はじめに

　胃レ線検査とくに精密検査にあたって，二重造影法による描写は，反対側の胃壁の皺襞にあまりさまたげられることなく，病竈をほぼ忠実に撮影されるので，きわめて有利であることはすでにひろくしられている．この考え方にさらに胃壁の伸展度の因子を加味すれば，凹凸についてはほぼ完全に描写されることであろう．

　このような考え方はレ線的にみた胃の各部分についてみると，まず後壁，次に，前壁，さらに，体部小彎について，発達してきたようである．

　後壁については本邦においては，白壁，市川，熊倉らが古くから努力され，完成の域に達したが，反面，白壁らは近年，胃壁の伸展度について，かなり神経質になっているようで，近年，とくに「空気中毒」なる用語さえあらわれてきたことは，よろこばしい現象である．このことは内視鏡の分野にも近年ひろがりつつあり，とくに微細病変をみようとするファイバースコープの立揚でも，やかましくなってきた．

　前壁については，とくに熊倉らは詳細な発展を度重ねて行なっている．

　体部小彎についてはSchatzkiがその描写法を発表してから，しばしば意識して行なわれるようになった．

　しかし，ここにのべる噴門部，体（上）部大彎側の皺襞の正面像の二重造影法による描写法は意識的に行なわれたものは世界中でないと思う．

　そこで以下これらについてのべるが，その部の病変はあまりしばしばないので，利用頻度は少ないと思うが，少しでも病変を疑った時は必ず，こころみるべき方法であろう．

　So far as the author has knowledge of, no one has yet reported on the method, by means of the clouble-contrast method, of drawing up the face-on view of the folds of the gastric corpus on the side of the greater curvature.

　And so this is an attempt to make public, for the first time, the details of the method as the author has carried it out.

　The region along the greater curvature is generally believed to be far less vulnerable, pathologically, in comparison with other areas of the stomach. But when you are in doubt as to the condition there, this method should prove invaluable for diagnosis.

　For, even with the use of endoscopes the diagnosis of lesions in this area is far from easy, the reason being the limited and partial scope of the pictures. With the use of X-ray pictures, however, there is a strong possibility that uneven irregular patterns of much wider area can be readily obtained.

　Now that the author has taken the first steps in dealing with lesions in the area along the greater curvature, my desire is to proceed in the direction of diagnosing even minute changes in this region.
