早期胃癌臨床診断の実態

Present Status of Clinical Diagnosis for Early Gastric Cancer

　早期胃癌臨床診断の実態の調査に先立って，集計に用いた病変の診断成績の解析にあたり，われわれの取った基本的態度の幾つかについてまず説明を加える．

　a）臨床診断はX線および内視鏡診断のlevelに止め，細胞診および生検組織診成績を導入しなかった．したがって，診断の経過については，初診時の検査成績の記載を重要視し，臨床的に経過を追った症例では，その経過中一度でも早期胃癌を考えたものについては早期胃癌疑いとして記載した．

　The rate of accuracy in the diagnosis of early gastric cancer was 60 per cent in 290 cases (307 lesions) so far encountered by the authors.

　Of these, 69 lesions (22.5％) were diagnosed as advanced cancer prior to operation and later confirmed as early cancer, and 17 cases (5.5％) were initially regarded as benign lesions to be established as early cancer by cytology and histology of biopsied specimens.

　In the stage of clinical diagnosis, 363 lesions were labeled either as early gastric cancer or its suspect, and 186 (51％) lesions of them proved to be so. The positive rate of early gastric cancer was thus 51 per cent. Lesions initially regarded as early cancer and later diagnosed as advanced one were 66 (18.2％).
