大腸に進行癌，早期癌，ポリープの多発性病変を認めた1例

Multiple Primary Carcinoma of the Large Intestine, Report of a Case

　われわれは，盲腸と横行結腸に進行癌，直腸に早期癌，S状結腸に腺腫を認めた1例を経験したので報告する．

　症　例

　患　者：33歳，男．

　主　訴：右下腹部痛．

　家族歴：伯父，母，兄弟に大腸癌を認める．

　現病歴：1978年12月31日，右下腹部痛を覚えた．発熱はなし．便の回数変わらず．1979年1月4日近医を訪れ，虫垂炎と診断され，虫垂切除術を受けたが，回盲部に腫瘤を触れるとのことで，回盲部腫瘍の疑いで当センターを紹介された．

　A 33 year-old man complained of the right lower abdominal pain. Four lesions were detected as follow: annular carcinoma 4.5 cm in diameter in the cecum, ulcerated protuberant carcinoma 4.0 cm in the diameter in the transverse colon, semipedunculated adenoma 2.5 cm in diameter in the sigmoid colon and early rectal carcinoma (semipedunculated polyp) 0.5 cm in diameter.

　In pathological examination, tumor of the cecum revealed undifferentiated adenocarcinoma; lesion of transverse colon showed well differentiated adenocarcinoma; adenonoma of the sigmoid colon showed moderate atypical epithelium and rectal tumor disclosed well differentiated adenocarcinoma in a part of adenoma.
