 This is a case report of 46-year-old male with massive hematemesis due to “Exulceratio simplex”, and with successful surgical treatment in Tokyo Metropolitan Bokutoh Hospital. The characteristic features obtained from this particular case are summarized as follows:

 1. Diffuse atrophic change of the gastric mucosa was seen in the fundic gland area.

 2. A Ul-Ⅱ ulcer of the posterior wall of the body was identified as the bleeding point.

 This ulcer caused erosion and rupture of the abnormally dilated artery which ran through the submucosa.

 3. The abnormal artery had its origin in the arteria gastrica sinistra and ran torturously in submucosal layer keeping the same caliber up to the bleeding point.

 4. The rupture was located just proximal to the portion where the artery formed its intramural arterial plexus.

 5. Because of this abnormal artery, slight papillomatous elevation of the mucosa was observed above it. Furthermore, the ruptured portion of the artery protruded into the gastric lumen. Therefore elevated mucosa with the ruptured artery in the center formed a polyp-shaped configuration.
