 In 58 cases of depressed or excavated types of early gastric cancer such as Ⅱc, Ⅱc+Ⅲ, Ⅲ+Ⅱc and Ⅲ, their excavation has been studied from the aspect of gastric acidity. As compared with Ⅱc type, the group of Ⅲ type (Ⅱc+Ⅲ, Ⅲ+Ⅱc and Ⅲ) showed in many cases either normacidity or hyperacidity of the gastric juice, a fact seen in higher rate in poorly differentiated adenocarcinoma. This result seems to suggest that in early gastric cancer, especially in poorly differentiated adenocarcinoma with normal or excess acid in the stomach, excavation be caused by secondary ulceration of the preexisting cancer lesion. Possible ulceration of cancer lesion due to the gastric juice is considered much more likely in cancer lesion with Ul-Ⅱ type ulcer within it, the more so when reports of endoscopic observations for Ⅱc lesions exterding over a long period are taken into accont. On the other hand, when ulcer of either Ul-Ⅲ or Ul-Ⅳ accompanies Ⅲ+Ⅱc or Ⅲ type cancer, the possibility of ulceration antecedent to cancer cannot be entirely ruled out.
