Colitis cystica profundaは比較的稀な良性の大腸疾患で,そのほとんどは直腸に発生する.その臨床症状および所見から悪性腫瘍として広汎切除を受ける例も少なくない.著者らは最近S状結腸に発生し,いわゆる大腸早期癌との鑑別を要した1例を経験したので若干の文献的考察を加え報告する.
Colitis cystica profunda is a rare disease, and only 12 cases have been reported up to the present in Japan.
The patient, a 48 year-old male, had complained of occasional lower abdominal pain for the past three years. Physical examination and laboratory studies on admission revealed no abnormality. On barium enema, a IIa+IIc-like lesion of 3.00m in diameter was found in the sigmoid colon. Endoscopically a caterpillar-like shaped tumor with superficial ulceration was found and the covering mucosa was edematous and hemorrhagic. The biopsy specimen showed normal mucosa with unknown substance in the submucosa. The resected specimen revealed a nodular elevation of 3.0×1.0×0.5 cm with superficial ulceration. Microscopically, many cysts covered with columner epithelium without atypia were seen in the submucosa. Fibrous stroma with lymphocytic ration surrounded them.
From these findings the case was diagnosed colitis cystica profunda.
