生検で消失し再出現後に切除した微小食道癌の1例

Minute Esophageal Cancer Which Disappeared After Biopsy, Reappeared to be and Resected, Report of a Case

要旨　われわれは，内視鏡と生検で確認した小さな食道癌が，生検後極めて不明瞭となった症例を経験した．その後の経過観察で，再び同部位により小さなヨード不染病変が明らかになり，内視鏡的粘膜切除術で切除し，ep癌を確認した．生検で小病変が変化することには注意が必要である．慎重な経過観察のうえ，悪性が疑われれば粘膜切除による治療が有用となろう．

　We experienced a case of minute esophageal cancer detected by endoscopy with iodine staining method and confirmed by biopsy. But the lesion became unclear when we tried to resect it after the biopsy. A smaller lesion reappeared at the same location two months after the biopsy, and it was resected by endoscopic mucosal resection later squamous cell carcinoma in situ of 2 mm in size was confirmed pathologically.

　We would like to emphasize that minute esophageal lesions may undergo great changes after biopsy and careful endoscopic follow-up without biopsy is sometimes important in equivocal cases. Endoscopic mucosal resection may be a useful method to deal with such a small lesion suspected of being malignant.
