 We experienced a case of minute esophageal cancer detected by endoscopy with iodine staining method and confirmed by biopsy. But the lesion became unclear when we tried to resect it after the biopsy. A smaller lesion reappeared at the same location two months after the biopsy, and it was resected by endoscopic mucosal resection later squamous cell carcinoma in situ of 2 mm in size was confirmed pathologically.

 We would like to emphasize that minute esophageal lesions may undergo great changes after biopsy and careful endoscopic follow-up without biopsy is sometimes important in equivocal cases. Endoscopic mucosal resection may be a useful method to deal with such a small lesion suspected of being malignant.
