 Sixty-one gastric leiomyosarcomas have been assessed for 10 histological parameters and DNA flow cytometry using paraffin material. The tumor size at its maximum diameter was the most significant factor for prognosis. Those of large size (>30 mm) indicated malignant behavior. Tumors with high mitotic count (3/25 mm2) behaved in an almost uniformly malignant fashion. The MIB-1 labelling index had moderate correlation with the mitotic index, however the difference of MIB-1 labelling index was not statistically significant for prognosis. By only the analysis of DNA flow cytometry, the presence of DNA aneuploidy didn't have a particularly strong correlation with prognosis. Immunohistochemical p53 positivity correlated with malignant behavior. It seems that the mitotic index, the DNA aneuploidy, and p53 immunoreaction are prognostic variables for gastric leiomyosarcoma.
