胃・小腸・大腸に潰瘍病変を呈した消化管原発T細胞悪性リンパ腫の1例

Primary T-cell Malignant Lymphoma of the Gastrointestinal Tract with Multiple Ulcerative Lesions, Report of a Case

要旨　患者は69歳，男性．心窩部痛を主訴に松永内科胃腸科を受診した．胃内視鏡検査で多発性の潰瘍病変を指摘され，発熱および体重減少を認めたため当科へ入院．精査で胃，小腸および大腸の広範囲にわたり潰瘍性病変を認めた．生検組織でT細胞悪性リンパ腫と診断され，Dawsonらの診断基準を満たすことから消化管原発の悪性リンパ腫と診断した．消化管全体に，広範に潰瘍性病変を認めた消化管原発のT細胞悪性リンパ腫の報告はまれであり，今回報告した．

　We reported a rare case of primary T-cell malignant lymphoma of the gastrointestinal tract. A 69-year-old male complaining of epigastralgia visited our hospital for further examination. Multiple ulcerative lesions of the stomach, small intestine and colon were detected by radiographic and endoscopic examination. Histolopathological examination of the biopsy specimen from gastric and colonic mucosa showed non-Hodgkin's lymphoma of diffuse medium-sized cell type. Immunohistochemical staining demonstrated that these consisted of UCHL-1, MT-1 positive, and of L-26, MB-1 negative. According to the definition of reported by Dawson et al., this case was diagnosed as primary T-cell malignant lymphoma of the gastrointestinal tract.
