 Forty-seven patients with distal (left side and proctitis) colitis were examined using total colonoscopy with regard to skipped lesions of the appendiceal orifice endoscopically and histologically. Sixteen patients (34.0%) of the 47 patients had skipped lesions around the appendiceal orifice. Comparing background features of patients with and without this appendiceal lesion, we could detect no difference in age, activities, duration of the disease, location of the lesions, clinical course, and severity. However, there was correlation between endoscopic grades of the lesions at the rectum and at the appendiceal orifice using Matts' classification. Six patients (37.5%) of 16 patients with appendiceal lesions showed an extended lesion from the appendiceal orifice to the ascending colon. Therefore, we suggest that appendiceal lesions in distal colitis are not infrequent and they probably extend to colitis on the right side.
