Diagnosis of Superficial Esophageal Cancer with High Resolution Endoscopy

要旨　現在，“高画質内視鏡”という用語に一般的な定義はなく，企業，団体によりその解釈は異なる．本稿では各メーカーの定義・高画質内視鏡の説明，画質の違いについて解説する．また，高画質内視鏡に加え，狭帯域内視鏡システム（narrow band imaging ; NBI）の登場により，低侵襲かつ高感度に食道表在癌のスクリーニング検査が可能になった．高画質内視鏡を用いたNBI観察では，“境界明瞭な茶色の領域”と“不規則な微小血管パターン”の2つの所見を診断の指標にすることで食道表在癌の診断がより容易にできるようになった．

　At this moment, there is no definition of the term of“high resolution endoscopy”and then, there is a discrepancy in interpretation among the companies of medical endoscopy. Herein, we explain the present state of the understanding and clinical usage of “high resolution endoscopy. NBI（narrow band imaging）combined with high resolution endoscopy provides better ability of diagnosis of superficial esophageal cancer. NBI findings of “well demarcated brownish area”and “irregular microvessel pattern”make it easy to diagnose superficial esophageal cancer.
