collagenous colitisの臨床病理学的特徴，病態発生と鑑別診断

Collagenous Colitis: Clinico-pathological Characteristics, Pathogenesis and Differential Diagnosis

要旨　CC（collagenous colitis）と診断した24例（平均69.8歳，男女比1：3）を対象とし，臨床学的所見と病理組織学的所見を解析した．また，CCを疑ってコンサルテーションされた症例のうちCCではないと結論づけたものの病理組織像を検討した．症状は下痢が20例，便潜血陽性が2例，症状なしが2例であった．剖検例を除く23例の通常内視鏡所見をみると，19例（82.6％）に何らかの異常が記載されており，A群“特に異常なし”4例，B群“軽度の変化”10例とC群“縦走する粘膜欠損・潰瘍（瘢痕を含む）”9例に分類した．LPZとNSAIDs内服例の内視鏡所見に着目すると，NSAIDs（－）/LPZ（＋）は10例で，このうちA群，B群，C群はそれぞれ1例，5例，4例であった．NSAIDs（＋）/LPZ（＋）の5例では，それぞれ1例，3例，1例であった．また，NSAIDs（＋）/LPZ（－）例は3例でいずれもC群であった．collagen bandの厚さは平均47μmで，LPZとNSAIDsの服用の有無による差はみられなかった．単核球浸潤スコアは3点満点で平均2点を超えていたが，全例で好中球と好酸球浸潤を伴っており，好酸球浸潤（1.2点）のスコアが好中球（0.7点）より高い傾向にあった．lymphocytic colitisの基準を満たす症例は認められなかった．これらの結果と文献的考察から，肉眼像の類型化を試み，病態発生を考察した．また，病理組織学的鑑別診断の問題点を明らかにした．

　To clarify the clinico-pathological characteristics and pathogenesis of collagenous colitis, we analyzed clinical and pathological findings of 24 cases（mean 69.8 years old, m : f＝1 : 3）of collagenous colitis. Symptoms of the patients were diarrhea（20 cases）and positive occult blood test（2 cases）. However, two patients had no symptoms. In 19 of 23 cases（82.6％）, abnormal endoscopic findings were reported, such as “minimal changes”（10 cases）and “linear mucosal defect or ulcer”（9 cases）. Among 9 patients with linear mucosal defect or ulcer, 8 patients had a history of taking lansoprazole and/or NSAIDs. Not only the deposition of a collagen band but mononuclear and polymorphonuclear cell infiltration with degeneration of the superficial mucosa have been demonstrated to be important findings for a correct differential pathological diagnosis. We attempted to typify the macroscopic findings and discuss the pathogenesis of collagenous colitis.
